Extra address and custom field display in search box

Hi,

 

I am trying to get some custom fields to display in the Search box (Country, State, City) to start with. I have tried adding the fields to the "Comma separated list of custom fields to show (i.e. jr_state,jr_city)" but they don't show.

I tried to Enable Module Theme and pasted the examples from the documentation but this removes the existing default search fields (which I want to keep). 

What is the best way to display the additional address fields and a couple of my own?
