For example, my client requires I create something like: For the Hospital/Medical Center listing type, he would like to collect: Hospital or Medical Center ACCESS: Ease of registering at hospital Parking access WEBSITE - I WAS: Able to sign up on the website for updates on my disease or medications Able to easily navigate the website Get everything I needed for my diagnosis and treatment SUPPORT STAFF: Seemed to have an understanding of my disease Seemed to understand the journey I'm going through Understood my loved ones' concerns Provided me/my family with emotional support Took the time to help me through my ordeal Seemed to understand the treatment I'm undergoing Provided me with easy-to-understand education Provided me with other reputable resources I can go to HEALTHCARE PROFESSIONAL TREATING ME: Was able to clearly explain my condition and the treatment Seemed to understand the journey I'm going through Understood my loved one's concerns Provided me/my family with emotional support Took the time to help me through my ordeal Seemed to understand the treatment I'm undergoing Provided me with easy-to-understand education Provided me with other reputable resources I can go to FACILITY: Had all or most of the things I needed to be treated Parking access to treatment/offices convenient POST TREATMENT FOLLOW UP - I WAS: Informed I would be contacted for a follow up visit (if necessary) Provided with information describing my next steps BILLING - I WAS: Provided with a clear understanding of the costs associated with my treatment/medications Directed to charitable institutions/foundations to help with costs Directed to Patient Assistance Programs (PAP) to help with costs Directed to pharmaceutical/healthcare companies for discounts/co-pay card discounts Provided with a person and contact information at the facility to help me with billing So there's actually 30 criteria, but organized within groupings.
