Creating Formbuilder template files

I am attempting to create new Formbuilder template files that will be selectable from the advanced options drop down menu.

 

I have created the json file and placed it:

 

/jreviews_overrides/views/themes/customtheme/fields_formbuilder

(also tried: wp-content/plugins/jreviews/jreviews/views/themes/default/fields_formbuilder)

 

 

I then created the output them file in the below:

 

/jreviews_overrides/views/themes/customtheme/fields_phpformat

(also tried: wp-content/plugins/jreviews/jreviews/views/themes/default/fields_phpformat)

 

Both files had their own name and title but for some reason it does not appear in the drop down menu to be selectable so it can be loaded. Is there another file that needs to be edited to ensure it appears and is selectable? I have tried clearing the jreviews cache.
