Hide Custom Fields for Free plans

I have 3 plans Free , Premium & Upgrade.

 

There are various custom fields for the category,

 

For jr_mobileno & jr_website , i only want to show the output if the plan is Premium or Upgrade.

And if the plan is free i want to show the output of the field as "***This is a Free listing***"

 

Also I want to "Send Inquiry" pop-up Button only for Premium & Upgrade plans.

I tried following - http://docs.reviewsforjoomla.com/?title=How_to_use_custom_variables, this is honouring the enabled plan form, but the whole form is being shown instead of "send Inquiry" button. ( Basically i want to show the nquiry form in pop-up as in default)

 

Can you please guide me on the above.
