Duplicate Meta Tags

Google is giving my site a hard time because of Duplicate Meta tags. For instance this URL:

http://www.concretedisciples.com/skatepark-directory/united-states-america/skateparks/rhode-island_c209/

 

give this many variants that all end up with the same meta. Is there a documented way prevent this from happening? I have 1,331 URLS with this problem...
