Urgent help required with JReviews configuration and template tweakage

I need some urgent help on JReviews as follows:

 

1. the ability for reviewers to add photos (just when they are reviewing, not adding a new listing)

2. integration of Google Maps (an easier alternative to people copying and pasting the embed code into a listing)

3. offer reviewers the option of choosing from a jump menu (many shops to be reviewed will have multiple branches so we want to prepopulate the menu with all the branches to avoid duplication etc.)

 

I am currently skinning the template and it's a bit trial and error as I've little experience with Joomla, so any help there would be appreciated too. The URL of work in progress is:
