Ratings/scores with fields instead of reviews?

I would like to have a rating system for a specific thing/location. Since this does not work for me with the normal review due to the structured data for reviews, I would like to set up something of my own in the form of a grading system.

What is the best way to do this with which field options?
 

I have 4 questions and users should rate each from 1-6. At the end, an average grade should be determined, then rounded up or down.

However, the individual rating should also be shown on the details page.

I thought of radio buttons (1-6) to click on in the style of a box. But how do I best combine this?

I appreciate any ideas.
