User review uploads resulting in 500 errors?

I've got some registered users trying to upload media comments here and they are getting 500 errors or 401 you don't have access to perform this action when they arrive at

In the access settings, I thought I gave the registered users proper access to do these uploads...

I don't get this error when I do uploads as a superuser. What am I missing here?
