SEO manager in Listing Type custom field by using the syntax

Hi is me again. I'm a SEO specialist, so I understand a lot about the subject. 

In the project I'm working we wnat the listing to show in the H1 and the Page Title:  {title} Medic {jr_category} {jr_estate} {jr_district} United States

So in the SEO MANAGER I'm putting that information, when I click in Apply to Listing Titles, it does the job but, at the same time it applies the same to the breadcrumbs, also in the category blog with all the listings, the problem is it repeats the category name field, the state, the district and the name of the country many times in the blog page of the category, this creates SEO problems because the same words a repeating so many times. 

Is there a way to show:  {title} Medic {jr_category} {jr_estate} {jr_district} United States only in the listing page but in the Breadcrumb and category blog only the {title} of the listing?
