Everywhere addon stopped working after update

Hey guys,

After we updated to the latest version our custom written everywhere addon stopped working.

 

When we go to the addons->everywhere page we even don't see it in the list.

I've cleared the cache and the file registry, but nothing shows up. Any ideas what could be wrong?
